We are in network providers with Moda Health Plan and Pacific Source Commercial Health Plans

 

Do you have benefits through another company? Many patients are still able to receive partial reimbursement depending on their plan. We provide a superbill after your visit that you can submit to your insurance company.

• FSA (Flexible Spending Accounts)

We recommend contacting your insurance provider directly to ask about:
